Why Your Labs Are Normal But You Feel Terrible

There is a conversation happening in thousands of doctor's offices every day. A patient — usually accomplished, driven, somewhere between 38 and 58 — sits across from a physician holding a stack of bloodwork results. The physician looks up and says the words that have become a kind of modern cruelty:

"Everything looks normal."

The patient nods. Thanks the doctor. Walks to the car. And sits there for a moment wondering why, if everything is normal, they feel so profoundly not normal.

Fatigue that sleep does not fix. A mental fog that coffee barely lifts. A body that used to recover in a day and now takes a week. Libido that has quietly exited the building. A weight around the midsection that wasn't there five years ago and refuses to leave despite the early mornings and disciplined eating.

Normal.

The Problem Is the Reference Range

Conventional laboratory medicine is built around a statistical construct. When a lab prints "normal," it means your result falls within the range occupied by 95% of the population tested at that facility — which includes people who are sedentary, inflamed, sleep-deprived, metabolically stressed, and medicated.

Normal, in other words, means average. And average, in the context of modern health, is not well.

Consider ferritin — the storage form of iron. A conventional lab flags deficiency below 12 ng/mL for women, 30 ng/mL for men. Functional medicine practitioners look for 70–100 ng/mL for optimal energy and thyroid conversion. A result of 35 is "normal" on your report. It is also a meaningful contributor to the fatigue you cannot explain.

The same dynamic plays out across dozens of markers:

  • TSH (thyroid): Conventional range 0.4–4.5 mIU/L. Functional optimal: 1.0–2.0. A TSH of 3.8 is "normal." It also means your thyroid is working hard to produce enough hormone — and likely failing at the margins.

  • Vitamin D: Labs flag deficiency below 20 ng/mL. Functional target for immune, neurological, and hormonal health is 60–80 ng/mL. A result of 28 is reported as normal. Your immune system, mood regulation, and testosterone synthesis disagree.

  • Fasting insulin: Rarely ordered in a standard panel. When it is, the reference range allows up to 25 mIU/L. At that level, insulin resistance is already well established. Functional optimal is below 8.

  • Homocysteine: Not always included. When elevated above 7 µmol/L, it signals methylation impairment — affecting everything from energy production to cardiovascular risk to mental clarity. The conventional upper limit sits at 15.

Conventional labs ask: Is something wrong enough to treat?
